The purpose of the Transitional Care Nurse is to focus on the management of chronic disease and conditions within the facility to reduce hospital readmission rates in accordance with accepted standards of practice, state and federal regulations, and operational policies and procedures. 

We’re looking for an RN or LPN with a compassionate mindset — self-starter who values work-life balance and appreciates the flexibility to choose shifts that fit their lifestyle. • Active LPN or RN state license and associated degree.
• Experience in a hospital setting is acceptable, long term care and/or case management experience, preferred.
• Certification as a Case Manager, preferred.
• A passion for providing compassionate care to senior residents.
• Experience in long-term care is a plus, but not required.
