Behavioral Health Care Manager

About the Role
The Behavioral Health Care Manager is responsible for the coordinator of care for behavioral health patients. The Behavioral health Care Manager contributes to the improved healthcare outcomes of the patient, increased patient engagement, and promotion of cost effectiveness.
Compensation & Benefits
Salary: $60,000 - $70,000 annually
Paid Time Away: 3 weeks (2 weeks + 6-7 federal holidays)

Responsibilities
Reviews patient files and Registry information to make appropriate Care Plan recommendation.
Effectively communicates patient issues and care plan recommendations to Provider and Psychiatrist plus implement their recommendations.
Promotes patient engagement and robust education.
Provides leadership for CoCM team members to improve patient outcomes.
Performs psychosocial assessments using evidence-based, standardized behavioral health tools and protocols.
Participates in Care Team Conferences.
Qualifications
Bachelor’s Degree (Required),
2+ years of experience in behavioral healthcare management (required)
Strong knowledge of federal and state programs
Bilingual candidates encouraged to apply
